How to get a Daviess County, MO, Library Card

NEW PATRONS

Library cards are free to all Daviess County, MO, residents and property owners with proof of residency or property ownership. 

  • Complete application form in person at either library 
  • Provide current photo identification and proof of current Daviess County address (either valid driver’s license w/ Daviess County address OR valid/current photo ID and piece of official mail, rent agreement, tax receipt, etc)
  • 1 reference contact unless living in Daviess County less than 5 years, then a 2nd reference is required
  • Patrons under the age of 18 will be required to obtain the signature of a parent or legal guardian to obtain a card.  Parents/legal guardians are responsible for materials checked out on their child’s card and information accessed on computer workstations.  Non-resident cards are available for an annual fee of $20.
  • Sign the card
  • Patrons applying for a new library card may check out 2 materials throughout a 90-day “New Member” status.  After 90 days, assuming patron remains in good standing regarding renewed and returned books, status will be changed to “Regular” with the following standard lending and renewal periods.

LOST OR STOLEN CARD:

If a patron's library card is lost of stolen, the library should be notified immediately.  A patron is responsible for all materials checked out on their library card if the card is lost or stolen and not reported.  A patron will be charged $1.00 for a card replacement.
